I think the flow of the event request form doesn't make sense – for example, "please contact us before reserving a venue" shouldn’t come at the end of the form.

What if the form is broken into several pages, with a next button and a progress bar? 1 page each for contact, event time, location, etc. Then you can put notes under each section.
a. Like in the Details page, add note: “Please note in the details section if the event times provided include setup/teardown time for AB Tech, and how much time is allocated for AB Tech. We will provide an estimate time required for setup/teardown based on the event request. (this section should really go under “End Time”, but seems harder to add a text box there than just using the details section)

I am hesitant to add the pages only because it add complexity for things that can break. Right now it's a simple, single form. A compromise would maybe be drop-down tabs that expand and collapse to split it into sections. That way we don't have to change how the data is stored or send to Tracker. You'll stay on one page but it will be less daunting.

We can definitely add more fields though, so once split into drop-downs that wold not be an issue.
