"description" attribute not allowed on root element in XSLT

The plan has been for each .xsl file to provide a description attribute on the root element for use when building "index.html" files. This is not yet automated.

Need to refactor this idea before automating it.

Turns out, no additional attributes are permitted on the root element of XSLT files.

Can probably move this to a "description" element in the HTML heading.

Rather than add a new element, let's co-opt the existing html/head/title element from each HTML file. With a list of available HTML files, build a table of HTML file, last modified date, and title line.

If the title line is not available, use some default text such as a single non-breaking space
