Sidebar with form overview

We discussed adding a sidebar to data entry forms with the following items:

• A table of contents for the form. You can click an item to scroll to that section.
• Show the autosave status of the form here, so it's always in view. (Feature/save agent form #119 implements this status, but you have to scroll to the top of the page to see it.)
