Replacing Jotform with Google Forms

Currently the webapp is using jotform for when visitors see an issue with the Ward Leader data. We want to change this so then we use google forms instead. We Also want to make sure data is still being carried from the origin URL so the google forms is autofilled with page the error was found on. Example sites to help with this are as follows:
