Primer support for pragma(inline). Probably won't work unless bodies of external functions are generated as well, which would be part of the cross-module inlining support.

Unblocked by #606, will switch linkage back over to decl_one_only first though in a separate patch.

Cross-module inlining and pragma(inline, ...) support both sound great.
