Left a comment on the ticket: For existing life event forms, I cannot add criteria on the first level of existing or new parent/child relationships. A created a new "test" form and was able to create parent/child/parent/child, and save as draft. Not sure if it's a bug or if it's because it's not published.

Other two look good:
