Add some sort of way to recover lost request passwords

There are a few ideas for how to go about this:

1. Just use comment verification again
2. Allow admins to reset passwords manually (though there wouldn't be a safe way to deliver them to users)

Solution:
Add a new page for resetting a request password, similar to Special:RequestAccount where they enter their username and are given a verification code.
