And some tweaks and design elements like the rounded corners aren't working either, nor can I actually add the Userchrome toggle to the toolbar, clicking on the extensions button brings me straight to settings in a new tab.
Anyone here know what's going on?

Hello! I was experiencing the same issues (aside from the userchrome toggle and extensions button issues you had- mine were fine), but thought maybe it was because I was on 126 and not 123.
However, I did a complete reinstall of everything after downgrading to 123, and all of the issues I had still persisted. (No rounded corners, side bar being cut off weird, etc) so I don't believe that is the issue. Hopefully that info helps the author
Update: I figured it out, this is what the author was talking about when they said "ArcWTF will clash with the default Firefox system theme, dark themes that have light toolbar/light URL bar, and light themes with dark toolbar/dark URL bar."
The default theme my firefox is on is "system auto", switching to dark directly fixed everything. Now to play around with choosing my own colors!
