Add eligibility questions for childcare programs

We need to add a couple of questions to the app regarding childcare subsidy eligibility:

• Are you about to start new employment?
• Are you a newly established self-employed individual (business less than 12 months old)?

Currently, we don't have this type of data, but these scenarios might qualify individuals for subsidies under certain childcare programs (link). If the user answers yes to either of these questions, we can display a list of programs they might be eligible for and advise them to consult the authorities for further details.
