Build a simple form people can submit to, for new sites. Example:

Name: Nurses for Bernie
URL: nurses.forberniesanders.com
Contact: [email protected]

We can either follow up on these manually or just automate creating a new site & mailing out credentials.

Happy to lead the charge on this as time permits but anyone with cycles you're welcome to contribute.
