Just like we're doing with IP Addresses (adaa4c0, adaa4c0, 6d4983f), it would be useful to provide an option for linking back to additional information. For example in an area where folks are not familiar with OS specific patch steps a link could be provided (say to a wiki page) that steps through the basics.

Add very basic support for email header, footer

- Add EMAIL_HEADER, EMAIL_FOOTER conf file options
- Add example conf file to illustrate currently supported
  options. Future supported options should be added to
  the file to showcase their use.

Going to try for solving this through support for an optional header and footer. That's generic enough to solve this need in addition to allowing for Redmine include() macro calls.
