My computer doesn't have enough memory to run the 3D simulation so I can't comment on your 3D mesh, but the mesh looks alright. I found two minor errors in your input file according to your description of what you intend to achieve:

pre_blocks = fuel
in theNt
section because you have precursors in the fuel block only.block = fuel
in thePrecursors

The 2D example has no vacuum boundaries because it doesn't contain the top and bottom surfaces. I got k=1.561946, which would probably drop to 1.3-1.4 when you run the 3D …
