"Contact us about hyperscale" not working

Summary

In attempting to use the link to "Contact us about hyperscale" I get the console errors in the image below and can't close the dialog or submit the form.

Steps to reproduce the behavior

1. Go to https://ubuntu.com/server/hyperscale
2. Scroll down to "Your roadmap for hyperscale success"
3. Click "Contact us about hyperscale"
4. Select "Next" in the dialog window (doesn't matter if any checkboxes are selected or not)

Expected behavior

I expected to be able to submit some details requesting information about HPC clustering with Ubuntu.
