Paul and I met on 01/14 to workflow the attachments in Miro.

Prep before Attachments

• In application_operating_authority add field for Is the authorized individual driving a 16 or greater passenger vehicle? display rule if service type is Touring/Sightseeing
• In application_operating_authority add field for Is their any contractors hired for the purpose of operating authority? Y/N for all service types
• In company_authorized_person remove field Is the authorized individual driving a 16 or greater passenger vehicle? for Authorized Person section

Attachments

Business and Insurance

• Add List of Drivers attachment if 16 or greater passenger is yes
• Add Sample of Independent Contractor Agreement (optional unless they have contractors employed) see above

Authorized Person

• Remove all other than affadavit
• Add Photocopy FRONT of driver’s license (Class “B” or “C” commercial) if 16 or greater passenger is yes
• Add Photocopy BACK of driver’s license (Class “B” or “C” commercial) if 16 or greater passenger is yes

Vehicle Information

• Attachment is Certificate of Title OR Lease/Rental contract
• Attachment Vehicle Photo for Touring/Sightseeing if 16 or greater passenger is yes

Service Type

• Add display rule for Is operating 16 passenger

Does the operating authority fleet include a 16 or greater passenger vehicle?
If yes, the operating authority must provide a list of drivers in the application.
Does the operating authority employ contractors as part of the business?
If yes, the operating authority must provide a sample of an Independent Contractor Agreement.
