I'd like to work on this starting tomorrow! Do you need just straight code, or something additional to integrate with your site? Are additional frameworks (for aesthetic value) allowable (like Bootstrap, draw.js, etc) ?

@agentm83 it is a static site, hosted off of GitHub. It does use bootstrap. We have been using google forms (again no hosting, or Form as a service). What was missing is the older versions of Google Forms did not send an email alerting someone that the form had been filled in (at this point it would be [email protected], they now do. We do use Heroku or AWS low end servers that are free or close to it, so that is an option also.
