Add test cases to the PlanDefinition apply testing

The PlanDefinition $apply behavior is changing from flattening to preserving nested structure, which will affect CDS Hooks plan definition processing. Add test cases for recommendations 4, 5, 8, 10, and 11 to the PlanDefinition $apply test cases to ensure that when this behavior changes it does not impact production-track use cases in the opioid IG.
