When using the Virtual II emulator version 11.1 as an Apple IIe, running on a Macintosh running OS X Mojave, if you boot from ootw.dsk and either start with CHECKPOINT 1 or watch all of the INTRO MOVIE till the end, the following text is shown...

Pressing Return simply shows the previous animation screen again for a few seconds...

And then you get the PRESS RETURN TO CONTINUE screen again. It's an endless loop. Swapping out the ootw.dsk for ootw_side2.dsk does nothing. I tried it from the Total Replay 5.0b2 and it gives the same thing.

Hello! This isn't a bug. This is in fact how the game proceeds (in the original, too). Your guy teleports into that pool, and you are meant at that moment to start swimming upwards, immediately. It is not an animation, it is gameplay!

I recognize that there is no particular clue that you are now in control of the game at this point. There was no such clue in the original game, either. The "clue" is that you have just reached a game over screen, which should suggest that you just missed some opportunity to do something about it, and pressing return puts you back where you can avoid the game over. :)
