This page is open for editing from the community to collect examples of where things have gone wrong.

We currently have a list of items paired with the default checklist on the deon website. For the ones on the site, we try to focus on examples that really highlight one particular checklist item. We also intend to keep a high quality bar for the sources. They should be either an academic journal or a mainstream news publication that has its own reporting and research.

Since not every example will fit neatly into that framework, this wiki page is for making it easy to collect examples that demonstrate multiple items or don't yet have a source that makes the cut.
