Patient registers or logs in, then they can view their record, give access to a new doctor using the doctor's username, and remove access to their record for previous doctors.

Doctor registers or logs in from a separate URL, then they can view a list of patients who have given them access to records, select a patient to add a visit record, and fill in the note for a patient visit record.
