Tracheostomy Tube Replacement Procedure Note

Immediately prior to procedure a "time out" was called to verify the correct patient,site, procedure, equipment, and support staff. RT at bedside.

Indications for this procedure as well as risks, benefits and alternatives were discussed with patient who provided consent and agreed to proceed.

The placement was confirmed by chest X-ray. Tracheostomy tube was secured with ties.

The patient tolerated the procedure well with no immediate complications
