index: provide a way to fill gap in the middle

Right now, -l does current_time..db_latest while omitting -l does db_earliest..reference_tx. If a stakedex index -l command fails halfway, we need to be able to fill in the gap missing in the middle.

Provide a --start-from option that conflicts with -l that takes a transaction hash assumed to be in the db. The cmd will then fill start_from..db_latest_before_start_from_or_reference_tx_if_none
