It should be very clear to referees how many attempts they have left on a test

Referee has 6 attempts per test, we should show it in some way how may they have used so far.
Especially when all attempts have been used this needs to be clearly communicated.

Initial idea - add a new column to the tests table - if a user has tried the test and failed it should show attempts left, with <2 attempts in yellow and 0 attempts in red.
