State and Country info not transferring to CC #7

I am using FS Contact w/ the Newsletter addon to capture name, email, and snail mail addresses to CC. Your plug in is successfully transferring all but two of the address fields to CC. Attached are screen shots of the field setups, which appear to match the setups for other fields that are working properly. Suggestions?
