IMO, it is typically not common to raise new bugs for test executions against those already reported. Instead, users should be offered an ability to associate the failed execution to an already existing bug. The Add Hyperlink option is the current workaround however it is not a comprehensive solution as all associated executions are not captured at the bug level.
