provider name in chat panel model selection list

Is your feature request related to a problem? Please describe.
I often use different providers (eg. ollama, groq, etc.) for the same AI model, and it is difficult to know which model/provider combination is being selected in the chat panel, because then there are multiple entries with the same model name.

Describe the solution you'd like
It would be great if there was a way we could could provider information next to the model name being (or that has been) selected in the chat panel model selection list - even better (?) if we are able define a user-defined custom name while adding a model.
