Regarding failing actions. This may be helpful:

At present, only the user who triggered the action (or, in the case of a scheduled action, the last person who edited the schedule) receives a notification of the job failure.

Your interest in sending the email to multiple people is a very popular request, and hopefully we will be able to build that in. Given its popularity, it's not terribly surprising there are actions available to send emails:
