CSS doesn't have styles for manual TOC #36

If "TOC: manual" processing instruction is used, no formatting added to user-tagged TOC content because CSS doesn't have any rules for the Macmillan TOC styles.

User needed to use manual TOC because this is a collection and they wanted to include individual chapter author names in TOC.

I created a oneoff_pdf.css for this project so not a rush.

Attached oneoff_pdf.css file to fix this issue. We should incorporate this into the core CSS.
