XWord has some functionality to make up words if none are specified. I think this is meant for diagramless puzzles where the grid is being constructed dynamically. However, it also comes up a lot when solving variety puzzles (e.g. JPZs) that don't have conventional Across/Down cluing.
For example, in this Spiral puzzle, selecting Outward clue 21-17 and then clicking the middle square causes a "word" to be focused for the entire middle column, despite the fact that the middle square is part of a word in that same clue list. This also tends to happen a lot with barred cryptics which have more conventional Across/Down clues.
It seems like we should be doing more to avoid making up words for standard puzzles; clicking a letter elsewhere in the grid should select a clue in the same direction if one exists, or one in another direction if not; moving with arrows should similarly prefer real words that it can find to fake ones.
