The issue turns out to be in pci_remove_root_bus() which first calls pci_remove_bus() which frees the struct pci_bus when its struct device is released. Then pci_bus_release_domain_nr() is called and accesses the freed struct pci_bus. Reordering these fixes the issue. The buggy code allows a malicious user to pass arbitrary value of filter->splen/dplen. Except for initial reference, mddev->kobject is referenced by rdev->kobject, and if the last rdev is freed, there is no guarantee that mddev is still valid. Hence mddev should not be used anymore after export_rdev(). Since MOCK_HUGE_PAGE_SIZE was introduced it allows the core code to invoke mock with large page sizes. This confuses the validation logic that checks that map/unmap are paired. This is because the page size computed for map is based on the physical address and in many cases will always be the base page size, however the entire range generated by iommufd will be passed to map. Randomly iommufd can see small groups of physically contiguous pages, (say 8k unaligned and grouped together), but that group crosses a huge page boundary. The map side will observe this as a contiguous run and mark it accordingly, but there is a chance the unmap side will end up terminating interior huge pages in the middle of that group and trigger a validation failure. There is this reported crash when experimenting with the lvm2 testsuite. The list corruption is caused by the fact that the postsuspend and resume methods were not paired correctly; there were two consecutive calls to the origin_postsuspend function. The second call attempts to remove the "hash_list" entry from a list, while it was already removed by the first call. Fix __dm_internal_resume so that it calls the preresume and resume methods of the table's targets. If a preresume method of some target fails, we are in a tricky situation. We can't return an error because dm_internal_resume isn't supposed to return errors. We can't return success, because then the "resume" and "postsuspend" methods would not be paired correctly. Its used from trace__run(), for the 'perf trace' live mode, i.e. its strace-like, non-perf.data file processing mode, the most common one. Later, we also found[1] another function __blk_mq_update_nr_ hw_queues where we first freeze queue and then acquire the ->sysfs_lock. So we've also updated lock ordering in __blk_mq_update_nr_hw_queues function and ensured that in all code paths we follow the correct lock ordering i.e. acquire ->sysfs_lock before freezing the queue.
