Description

The CreateFacilityForm currently uses the outdated MultiOptionSelect component, which disrupts UI consistency with the other ShadCN components.

Expected

Develop a ShadCN-compliant MultiOptionSelect component and replace all instances of the old MultiOptionSelect in the codebase to maintain a uniform UI.

Hey Team, I would like to work on this feature. Already made the component just needs a little touch up and then good to go
