Need to determine official urls and publishing locations for IG

@brynrhodes indicates that the computable care guidelines will publish under Fhir.org. He indicates that this is open to other organizations (e.g. OpenHIE) if they undertake to maintain the guide. This will require coordination with @grahamegrieve. We should have a unified approach for all the case reporting IGs.

Once determined, review all canonical and other URLs/URI in the IG.
