  • Form is launched when the 'PNC danger signs' form is selected from the upper right corner of the profile page
  • Form should use a similar style as the above form
  • Back arrow at top of screen should take you to the previous page with a heading, "PNC danger signs"
  • If back arrow is selected, a confirm close message appears, "Are you sure you want to go back? All data you have entered in this form will be cleared" with choice options of "Yes" and "No".
  • If "Yes" is selected for the confirm close message, the filled form loses all the content in the answered questions and goes back to the previous screen.
  • If "No" is selected for the confirm close message, the filled form reappears with all the content in the answered questions prior to clicking the back arrow.
  • SAVE should save and submit the form the form
  • Change the form to JSON and verify that the form content matched data dictionary, Tab "HF1 - PNC Danger Signs" - XLSForm can be found here
  • Once the form is submitted, it will result in a notification to the CHW app specific to the PNC danger sign form
