International Patient Summary 2.0 Feedback

Version 2.0 of the International Patient Summary FHIR implementation guide is being worked on and feedback is sought by the international community. We would like to discuss here any issues or thoughts on how IPS may be achieved in Australian including use of AU Core/AU Base.

You can see a high level summary of constraints that need to be imposed when generating IPS conformant content from FHIR resources that are conformant to AU Core here: Comparison

Some terminology items raised and potential approaches or feedback already noted:

• Current Smoking Status as required binding - AU uses SNOMED (as does US). We would need to provide both codings when generating IPS conformant content.
• Observation profiles that mandate LOINC in Observation.code - we often mandate SNOMED in AU - We would need to provide both codings when generating IPS conformant content.
• To some degree we'll need to deal with https://build.fhir.org/ig/HL7/fhir-ips/CodeSystem-absent-unknown-uv-ips.html - we use SNOMED codes or Data Absent Reason
• We may need to shift Pregnancy Status as required binding - current draft AU uses LOINC but through consensus processes we may shift to use SNOMED (which is what is used in US). If we do then we would need to provide both codings when generating IPS content.

Things I think changes to IPS should include:

• DiagnosticReport.status to change to allow final, amended, corrected, appended. Noting in AU we do exchange interim reports. With the constraint excluding interim these reports would not be able to be shared in an IPS conformant patient summary. Question to AU group do we want preliminary supported?
