Rewrite tutorial

The documentation currently tries to build on the poll app from the default Django project, but there is too much setup and background. It makes sense if someone's recently done the tutorial and is going to be following along, but most people won't - it's just confusing.

The tutorial should start from a clean project - it can still be a poll app, but start from scratch.
