GetCombatants() doesn't return the correct party members after wipes

I looked into the party list issue on Triggernometry since 5.2. It seems after several wipes in dungeon, the GetCombatants() method fails to return the correct list of party members (in 5.2 and 5.3 it will return a list with repeated members, and in 5.4 it will return a list with some members missing from the list).
It seems to me this is the issue with FFXIV Plugin since old version of Triggernometry run into the same problem.
