Item order not preserved in JSON export

A user exported a complex study with both item_order and block_order set. Importing it from Google sheet worked fine, but when exporting to JSON and re-importing the order was not preserved (it was re-ordered according to item order which messed with the block order).

We actually have three order variables: index (order in which they are in the sheet), block and item order.
