Cumulus Library 2.1 release

You can install the latest library release with pip install -U cumulus-library. The release notes are over on the project github.

We've added a way to create tables from static flat files, and have used that to make a UMLS study for creating tables from the Metathesaurus exports. You'll need an API key for this, see the study repo for more infomation. The hypertension study has also been updated to support the v2 library.

This also contains numerous bugfixes and improvements, especially to medication tables and denormalization. This should improve data quality for downstream studies, so we'd recommend upgrading to this version before your next library run.
