Chart Review 2.0.0 release

See the full release notes for more info, but basically:

• Moved the info subcommand's various functionality to other commands (ids, labels)
• Added new commands (frequency, mentions)
• Added a new --verbose flag to the accuracy command to see all the nitty gritty agreement data
• Added a new --csv flag to all options to see the output as a CSV

Run pip install -U chart-review to upgrade.
