I have an LG Leon LTE with Cyanogenmod 12.1. Your app is the only way I've found that actually succeeds to change the MAC address of my phone. The problem is that after the MAC is changed I can't connect to any network. I've tried disabling the app, reverting the changes, using different addresses...In the end, my WI-FI won't work again until I disable the app and reboot my phone. Any idea on why does that happen?

Is there a specific error you get? Do you try to connect and get an Authentication failed or are you not able to turn on wifi at all?
Please post a logcat of what happens when you try to connect to a network so that I can help fix this. Also you could try setting SELinux to permissive (run setenforce 0 from a root adb shell).
Same here. I have posted to XDA. This is a problem with CM 12 or something? I tried your app and other app, MAC was changed very good, I capture packets from Commview Wifi and noticed MAC was changed. However, for OPEN AP, client didn't got IP from DHCP. If client assigned static IP, did not ping or arp, no response from AP. For WPA2 AP, didn't pass 'Authentication error'My phone is Nokia X2DS CM12.1build5 systemless
