Support for notes

Summary

Allow for an series of notes that are linked to the user and ticket to be viewed and used by moderators. Notes can be only edited by whoever wrote the note down and can only be deleted by either who wrote them down or an admin. And notes can be viewed by all mods.

The Problem

Currently moderators don't have access to any previous documented context expect from those previous encounters.

The Ideal Solution

It would be ideal for moderators to be able to quickly look up any notes written by others on an particular person. This takes off the burden from moderators having to memorize what happened on what person. Thus, an notes system that is linked with the ticket holder is what we want.
