Having multiple blocks of titles

Hey there, Thanks for the awesome work on this.
I have a question, and I've been scratching my head over this.

How can I have multiple instances of Title per document? Something like (title block+)+. The purpose is to have multiple titles and convert any H1 that users input to a title node.

I was also wondering how can I add custom commands to the menus? Say if I have a custom node that I want users to be able to insert
