Backend controller user registration

Patient table will be getting updated when we are filling the user registration form from the frontend. Api routes has no change similar to all other routes.

Whenever new patient is getting is registered, we need to create a new user in the xceed user management.

we will adding username as their email-id, default password is 12345 and role as patient. This entry has to be done seperately in the xceed user table.
