WHAT:
Dashboard to show results from Self-declares.

PHOs can push requests to self-declare through notifications
or through an on-scan
by campaign, area or data provided. (dashboard)

They need to see the results of the self-declares by

ARea - via last check-in
combined symptoms
Age and other filters from data provided.
