Problem: GL-E703/A: default -> defaillance+Sex -> FAIL. We were checking the default category with displayName==="default", use name ==="default" instead (code cannot be used, it's not set by DHIS2).
Do we need to prevent non-EN users to create category combos? No, as we create the COCs manually (with English names)
Feature: add translation for new category combos (from categories[].translations)
