Page view measurement fixes

• Fixed incorrectly formatted raw page names and levels.
• Page views are now measured automatically only when a view appears because it has been added to a view controller hierarchy, not when it is revealed because some other view controller was dismissed. This is the only NetMetrix-compliant way to measure page views. In agreement with the SRG statistics team, this approach has been extended to comScore measurements as well.
