v1.0.2 Quick Fixes

Fixed the issue with generating the pngs with transparent backgrounds when they're resized to 48x48
Actually, added the code to generate the index.md file each time the sprites are generated

Fixed issues with entity and sprite generation

This should work for both sprites and entities within PlantUML documents. This follows the normal sprite format and also the tupadr3 entity format
