This release removes chain names from identifiers in the subgraph, instead it now uses URL routes to differentiate between chains. This mitigates the issue we encountered when changing the name of one network from xdai to gnosis-chain, wherein all commands referencing an existing post on on xdai were rendered invalid.

With this release, Tabula is finally production ready. 🎉
