MFD plugin for OpenSim 4.1 (VSC16.5.4)

The package include:

1. the plugin (.dll) compiled using Visual Studio Community 2019 (v16.5.4) for use in OpenSim 4.x
2. an updated version of the users' guide.
3. a shoulder model used in the users's guide example.
