Annotations with Annotator

All annotations are being handled by Annotator. The madison plugin for annotator can be found in /public/js/annotator-madison.js.

Right now annotations work as follows:

Anonymous users can view but not edit / create annotations
Logged in users can create / edit their own annotations and view everyone else's
A logged in user can upvote / downvote / flag any annotation
A logged in user can comment directly on annotations
