Update deps and add new rules

Added new rules, updated from 0.18 to 0.32 on Markdown lint.

Documentation improvements

Updated the documentation to be better for copy/pasting to a new project.

Initial Release

This has all rules and tests in place for this first version. I'm expecting to update it as markdownlint fixes bugs and implements features I've requested.
